Service requests
A free-form application for the issuance of a re-disability certificate must be written and submitted to the State Commission of Health and Health Examiner Doctors or the Department thereof. An application may be sent in a postal item or submitted to an e-service “Reissue of a Disability Certificate”Receipt of services
The prepared disability certificate shall be received in the presence of the State commission departments of the State Commission of Health and Health Examination Doctors or in a registered mailing
